Summary: Best you can get for the topic of CPET
Comment: I am working as a research fellow at the Harbor-UCLA Medical Center, and have direct contact with the authors of this excellent book. There are lots of results of original investigations by the authors, and I think they are the ones who know the most about this topic in the world! Almost all papers on CPET quote this book as a reference. If you are in the respiratory medicine specialty with interest in CPET, this is a must-read.
